Initial Comments:
An off-site, state licensure follow-up survey conducted on September 13 and September 14, 2023 found that Maylath Valley Health Systems, Inc. had corrected the deficiencies cited under 28 Pa. Code, Part IV, Health Facilities, Subpart F. Chapter 601. The deficiencies were cited as a result of a state re-licensure survey completed on July 14, 2023.
